Appointments
We ask that you would keep your appointments, arriving on time. If you should need to cancel or reschedule, we ask for as much notice as possible, but not less than 24 hours. There is a small fee for a broken appointment or late cancellation.
Children
We know that you love your children and we do too! If your child is not the patient, we prefer they do not roam between rooms unsupervised. We ask that they would either stay in the waiting room where we are able to provide limited supervision or sit in a chair in the corner of the operatory (age appropriate) with parent attention. If your child must use the rest room, we will ask a parent to accompany them.
Insurance
Please let us know your insurance information ahead of time and inform us of any changes. You may refer to your plan booklet for details of your particular plan. Our office will submit claims on your behalf. We ask for your co- payment at the time service is rendered and the prompt remittance of any remaining balance should the insurance pay less than expected.
Payment
Our office requires payment in full at the time service is rendered. We accept cash, check, credit card, money order or cashier's check. With verified insurance, we will ask for your co-payment and accept Assignment of Benefits. Benefits for cash patients who pay in full on the date of service:
5% discount
10% discount for seniors (age 65 or older)
Note: in order for this to be in effect, the entire balance must be current.
Payment plans
Payment plans must be adhered to, or no new appointments. Of course the doctor will see true emergencies, but only emergencies. If patient has large balance, but his or her insurance will pay 100% for cleanings, then cleanings can and should be rescheduled.
Care Credit
We are pleased to offer Care Credit, a private line of credit available for your dental or health needs. There is no application fee or yearly fee. Care Credit has a variety of payment plans including three different interest free plans. The application is simple and allows you to begin treatment immediately. |
